When One Tooth Changes Everything: Fabrizio’s Story

Fabrizio came to Sonrisa Italiana with a clear and heartfelt goal: “I just want all my teeth to look the same.”

He was about to embark on one of the most exciting seasons of his life: his honeymoon was just around the corner, and shortly after, he would be attending an international summit for startups—an event that meant a lot to him as a young entrepreneur who had just launched his own app.

For Fabrizio, this was more than just a smile update. It was about feeling confident, refreshed, and ready to show up—both personally and professionally.

Clinically, his case revolved around tooth 21, which had a visibly darker shade than the rest of his teeth. The discoloration was caused by trauma he had suffered as a child, followed by a root canal. Over time, the tooth had taken on a grayish-yellow tone that stood out in photos and daily life. Adding to this, Fabrizio had several malpositioned teeth, which disrupted the overall esthetic balance of his smile.

Upon examination, we also found old composite restorations on teeth 11 and 21, which had already exceeded their useful lifespan of 5 to 6 years. These fillings were stained, opaque, and no longer blended with the surrounding enamel. We removed them entirely to assess the original structure of the teeth and define the most appropriate plan.

The case began with a deep dental cleaning to prepare the soft tissues and eliminate any local irritants. Then, Dr. Alicia Correa, our periodontist, evaluated the gingiva and bone to ensure everything was stable and healthy before moving into the esthetic phase.

To address the darkened central incisor, we performed a walking bleaching technique on tooth 21, carried out by Dr. Laura Bula. This method involves placing a whitening agent inside the tooth and allowing it to work gradually, lightening the internal shade of the tooth from within. It’s a conservative, effective option for non-vital teeth with discoloration, and in this case, it helped establish a more uniform base tone before any restorative work was done.

The gums might look slightly affected in the photo due to their natural sensitivity—during the removal of excess material, some minor irritation can occur, but the tissue heals on its own very quickly.

The final touch came with 10 stratified direct composite veneers, crafted and placed by Dr. Daniel Zabaleta. This technique recreates the natural layering of the enamel and dentin, allowing light to reflect with nuance and softness—key to achieving a smile that feels real, not artificial.
